what I learned from watching a few of those episodes:
a woman doesn't necessarily have to be large to not know she was pregnant.
Some women experience "spotting" during their pregnancy, which can be mistaken for a period. The woman then will not practice any prenatal care which can hinder the size and growth of the baby (causing her stomach to be smaller than a known pregnancy) as well as a possible premature baby.

How does the girlfriend not call her boyfriend and say "Hey, I'm going into labor or a baby just came out of me" and ask for his help? or call 911? dang. Women giving birth can bleed to death. And her boyfriend is 30. I don't know how old the female is, but would a baby at 30 ruin your life? I know it was a surprise, it is just weird she didn't contact the boyfriend.

Boyfriend needs to leave her and testify against her. She needs to spend some time in jail.
