Sonnen is actually at a testosterone deficit due to the fact that he suffers from hypogonadism. He pretty much needs the treatment to survive. Prior usage of steroids was probably the cause of his hypogonadism, however.
Even if he needs it, his testosterone ratios were absurdly high (17 times that of the normal man and 4 times they allow an athlete) after he was tested from the last fight. I'm not doctor but that seems like abuse and exploitation of the TRT and an advantage in his favor. He got approval this time around, but I wonder if that means he has to keep the ratios within the allowable amount.
