The salaries still match since it is it their obligation at that point to turn the deal down and since they love Rudy so much they would not want to make CD change his mind by asking for Howard instead. The physical examination is there to protect the teams and if one team say that they are fine with just receiving an "injured" player then that is their prerogative. (see Orlando's deal with Sacramento where they admitted knowing that Doug Christie was not "fully healthy" but still allowed the deal to go through. There are more examples of this including IIRC the Rice deal to Utah).
