    You're right. One mistake out of millions of cases should definitely keep you from doing something that could save people once you've passed.

    Wait, what's even more rare that a mistake like that is her waking up just in the nick of time. In most cases (if this happens) I imagine the patient never wakes up and just dies from the operation.

    She heavily overdosed. Pretty obvious she wanted to die anyway.

    She went ahead and killed herself 2 years later anyway, so I'm pretty sure that was what it was.
