Apparently, he gets decapitated early in the film. The rest of the film is his body fighting ISIS to find and reattach his head. It's all that HGH has essentially made him indestructible. He just needs to sew his head back on and the HGH injections will heal him up (no scarring). The Headless Horseman has nothing on Headless John J Rambo.

Yeah, Stallone's camp already denied everything but the fact that they are working on another Rambo movie. Apparently Sly wasn't even AT Comic-Con.
And the last rumor I heard was it would bring his character back full circle...back to the US to re-connect with his father. That it'd be somewhat similar in tone to Rocky Balboa. But that was a long time ago I heard that.
He will be fighting homelessness and hunger right here at home in 'merica...defending a large homeless camp from brutal government thugs wishing to uproot them. "They drew last blood.
