Some did some didn't. They were paid by taxes like NOWADAYS in most countries in the world. And the private doctors could become doctors of the rich. As for life expectancy you know that there was a ton of child deaths and then every man had to fight wars every once in a while? However there are many historical figures who lived well into their 80s and 90s like Sophocles and Platon. As long as you were lucky not to die in war die from a plague face a famine captured and be enslaved the usual BC life troubles.
