Find him on one of the many "Find a Doctor" sites that I'm sure he's subscribed to and anonymously trash him - clever but not too harsh.
Add enough cryptic information so that he knows it's someone that knows him.

Wow, props for taking it so cool. My adrenaline is pumping just by reading your story but hey, I do have some anger management issues that can be dealt with reasoning and a nice blunt. Seriously you need to take a few deep breathes and let it go. If he vandelize your car then go to the police. Does the condo have security cameras? If not do you have assigned parking, if so mount a wireless camera .
If you get evidence & the cops dont do anything then you borrow a cordless drill screw all his windows & doors shut then mix a little coal oil and gasoline together and burn his house down when he is home that will teach him!
Be sure to buy the gas can and gas with cash a long way from home then dispose of the can back across town.

If you're shopping for a new primary care physician, keep looking. They've been consistent about getting me into the office as soon as I need to be, which is about the only positive thing I can say for them as apparently that was just a ploy to get as much money out of me as soon as possible. My insurance bill has consistently come back ridiculously high with over charges (double charges on procedures) not to mention some made up procedures. I should also mention that of the dozen times I've been, I've never seen Dr. Douche, apparently he's too busy. Wherever he is, I would recommend that he and his entire staff take an ethics class.
