When I was little I watched Lifeforce on cable. It was so cool watching an incredibly hot supermodel walking around butt naked for the entire flick. But when she sucked the life out of her first victim, turning him into a withered husk of his former self, I was seriously freaked. And the pant soiling commenced when the zombie/corpse came back to life an hour later and started sucking the life out of new victims. Aye caramba!
Yep, it was pretty scary. The only movie that ever gave me nightmares was Alien. Actually I never even saw the movie until I was older. All good examples, thanks for the drive down therapy lane. one I didn't see listed yet (may have missed it if so), when dude in Alien is feeling better and having breakfast before cryo, after having that thing on his face, starts choking, and then.... convulses, splat, EEeeeeeeEEEEeEe. Yikes. The look on their faces was the same look I had. Another? Half the scenes in Carpenter's "The Thing" but especially when they're tied up and testing each others blood to see if they're "infected." The Curby in AmHistX is distressing. I had heard about curbies before that, but it was the first time I'd seen a depiction of one. horrific. Old lady in the bathroom in the shining was also disturbing. I'll never forget that laugh with her rotting arms hanging outward. ewww. that scene of sloth-coughing in se7en is also very startling. Opening scene of Jaws I where the chick gets dragged about the surface of the water before being yanked down. I too was allowed to watch "The Exorcist" at wayyyyyyy too young of an age. It was on cable and I was staying at my cousins' house. They were older, and watching it. The dad (divorcee) was like, well, if you want to, ok. I didn't, but didn't wanna seem not cool. Mainly it was the thrashing about and the bed and furniture moving (combined with that voice) that really scared me. It freaked me out for years. After that, any movie where stuff moved around on its own (close encounters when he's in the truck and the mailboxes move) or anything remotely satanic, sent me into a subtle panic, and I was very skittish about the dark and my room, etc for a few years. Yes, I know, TMI... but age 7-8 is no age to see such things. I now have to watch it when its on... for whatever reason. I'm sure there's more, but those examples already listed are definitely tops.
Apparently the surprise was genuine as the actors were kept in the dark and didn't know what was going to happen in that scene.
I found "HellRaiser" disturbing [all things Clive] Saw movies Exorcist was terrifying . . .as was Amityville horror. Poltergeist was in the mix as well 30 days of Night was a GOOD Vampire Flick one of the best I have seen in a while. . . Rocket River Shane getting ratted out by the thug on the Sheild was an incredibly intense scene

I did not know that. cool. You see it a bunch of times and its like no big thing, but I remember seeing it in the theatre and was paralyzed with shock.
Stabbing scene from Zodiac. One of the few murder scenes that makes me feel truly disgusted and violated. The hospital scene in Jacob's Ladder (scariest movie ever). Also there used to be this Plantowsky's commercial where they would flash like a million pieces of furniture on the screen. Then at the end there was this dog sitting on a couch and this lady called to him, she had a really scary voice, then the dog jumped off and ran off screen. I usually ran out of the room when that happened. Plantowsky's was furniture store in Houston.
Yeah, I guess when you're five, watching this on a movie screen looks pretty scary. So does Large Marge in Pee Wee's Big Adventure.

The spider scene from The Thing was scary when I was very young but it'd probably look like cheesy sfx today.
