The poor have been punished since the dawn of civilization. In fact poverty was completely created after and by civilizations themselves. I think it's human (animal) nature; a sort of Darwinian effect that the strong get stronger and weak get weaker but with human created abstract concepts and human intelligence/ignorance thrown in. I don't think poverty will ever go away as long as this current species of humans exists. That doesn't mean we shouldn't deal with the problem. On the contrary, we should all do our part but understanding that it will never be eradicated.

Great, an artificial construct we created has declined because of additional ideas and concepts we've created. Seems a bit illogical doesn't it. Won't change the fact that for whatever reason hundreds of distinct human populations got together separately and at separate times and all said hey let's build a society where we're not equal!!! Those same groups of humans, although more enlightened and altruistic today, are trying to erase inequalities by creating even more labels and groups which divide humanity even further like Race, Color, Age, Sex, Language, etc... when you consider we're all Earthlings. Back to this Medicaid (or any "faux altruistic" program), it's seems irrational to try to fix equality and balance on top of a system that is inherently unequal. Like asking someone to solve calculus problems with only algebra maths/tools, impossible. It will be the first androgynous, transgender, altruistic, homogenized, brown hair, brown skin humans who might figure it out all out.
Poverty is a very subjective term. We like to half ass slap an income wage identifier to determine someones wealth and come up with excuses why lower income people can't make more. Unfortunately you can not have everyone "middle class". All society owes is to ensure everyone gets fed properly, a roof over their head and medical attention.

Yes you can. In addition, you can have almost unlimited variations of what proportion is middle class. 1%. 50%. 90%. You can have one obscenely rich person, one obscenely poor person, and 318,999,998 middle class earners. Anything is possible. There's no natural number. The way a society chooses to govern and treat its fellow humans decides 100% of the time what that number is going to be. It's entirely based on morals. You can have whatever you like. There's no default percentage. What's more incredible, there's no default way that being fed properly, having a roof and getting medical attention has to be supplied! It can be really ****ty versions of those things or really great versions. You know what else is incredible? You're right! Poverty actually IS a very subjective term, but not in any way that you described it in your post. For example, if income doesn't change but the price of food increases, then you are becoming poor even if you weren't poor to begin with. The relativity is important. In this sense, poor is relative. It depends on what you can afford. It depends on what new diseases become prevalent. It depends on what you need to eat to survive in this world. However, regardless of whether excuses can't be made for why a poor person is poor or why they're not making more money.... they are still poor. In that sense, poor is abso-****ing-lutely poor.
