I'm pretty sure Elle MacPherson's character never found out his identity. As for the rest, I guess they could keep a secret. The plothole that always bugged me was Drew Barrymore and Debi Mazar finding out in Batman Forever, yet there were no consequences from this.

I always thought that Ray Liotta would have been a good Joker if you were going for the crazy killer. Listen to his laugh in Field of Dreams when he talks about Ty Cobb; that is one crazy sounding laugh. Plus to give it a little extra realism all you would have to do is get him liquored up and then he could really pull it off
On second thoughts, Ledger wouldn't be bad for the role. If they made the Joker a transvestite it would work very well.
