Cap holds are accounted for, but 'only' for 100% of the rookie-scale. There's an unwritten rule (between agents and teams) that it's mandatory to sign your rookies to the maximum allowed 120%, which obviously increases the cap hold. Thus by not signing your rookies, you do preserve some cap room. It's not much, but I guess in some cases every bit counts.
