Do all confederations compete? Yeah. The winners of each confederation play, plus the home countries champion I believe. And I believe this is Real Madrid's first title.
Yeah it happens every year. All of the federations participate in it. It's prestigous for other federations besides uefa, just because no matter if they win it or not they are usually still recognized as the best team due to them being uefa winners.
