My guess is that the players thought they could win games just by suiting up for the game. The success they had last year went to their head.
No idea. Certainly, Harden may be having some personal issues, but that doesn't explain the collapse by the entire team. I definitely think McHale did a horrible job in the preseason in getting these guys ready. He made no effort to integrate Lawson into the offense. He didn't fix any of the defensive lapses we saw in those 8 games. He didn't figure out what rotations he wanted to use. He basically just wrote off the preseason and let a bunch of scrubs get heavy minutes before they were all cut or sent to the D-League. Maybe some of the guys tuned him out when they saw the lack of preparation. I don't think we have a team of quitters. Look at Game 6 vs the Clippers for God's sake. Aside from Smith, the rest of the players who mounted that comeback are still here. But maybe they just reached a breaking point where they realized McHale wasn't do his part as head coach and turned on him.
THIS. That family is just a circus. Why anyone would want to be embroiled in that drama I do not know.

Glad McHale is accepting accountability for the team's struggles. He simply wasn't clapping hard enough.
