(If you are under 18 years of age, your parent or guardian must sign the form for you.) In addition, please print this auditions page, fill in the requested information, and fax it to us at (713) 513-8301 before Friday, July 19. You will also need to bring this page with you to the auditions on Saturday, July 20. TIME AND LOCATION The Rockets will be holding dance team auditions at 8:30 a.m. Saturday, July 20 at Chancellor's Racquet & Fitness, 6535 Dumfries, Houston, TX. (To obtain directions to Chancellor's Racquet & Fitness, please call (713) 513-8311.) REQUIREMENTS Call the information line (713-513-8311) for more details Must wear flesh-colored tights or hose, two-piece leotard, and jazz or dance sneakers Must be 18 years of age by October 1, 2002 Must bring photos - head shot and full-length (non-returnable) Must bring a resume with personal and professional references Must bring a picture form of identification AND $10.00 application fee Must sign a release of liability form. If under 18 parent must sign. No spectators or guests. Parents allowed in to sign release form. No kidding. If there's going to be a dance team, I want it coed! (unless we're supposed to be looking at the basketball players, not that any of them do it for me ) Or at least show off some beefcake during Comets games.
