Pre-signed a card, put it in her mouth. Used her signed card and put it in his mouth. His other signed card was for show.
My guess is that he already has a King of Hearts with his signature on it somewhere in the deck (a duplicate). Using sleight of hand, he forces that card in the girl's mouth, with her thinking it's her own signature card. Meanwhile, the Ace of Hearts she signs is placed back somewhere near the top of the deck, and instead of putting the card he just signed in his mouth, he uses sleight of hand again to put her signed card in his mouth.
This is a pretty easy one. Spoiler He already had a card he signed at the top. He flips over the top 2 cards, she signs the face, and he flips them back over, giving her the top card (the one he signed). Then he flips over the top 2 cards. Signs, and flips them back over. He puts the top card (the one she signed) in his mouth.
