I get called this sometimes by guys who I sense are intimidated by me and are trying to knock me down. My wife equates it to little man syndrome. I think it's super passive-aggressive and disrespectful. "Champ" and "Boss" also apply. Any euphemism that is normally considered a respectful position turned on it's ear.
Depends who is saying it. Some people are oblivious that those are insults or may have been called those things themselves and thought the other person was being nice. Then passed it on. Especially foreigners who would have trouble understanding the connotation.
My dad calls everyone either Cheif or Boss. I thought it was just a old school thing. I think your being overly sensitive about it. More modern word for it would be, Man, dude, and Bro.
Just the opposite. It can absolutely be a sign of respect when said respectfully. Friends say this to each other all the time. If said disrespectfully, then it isn't. I don't think there's a hard rule with these words.

That's exactly what I'm talking about. Don't be a p***y about it, just come out and call me an a-hole. (I don't have subordinates. I steer clear of management positions. -shudder-)
