    Looks like a prank. Why would they be randomly filming? He starts off in front of the camera and works his way through the crowd just to be the last one on the subway. He purposely thrusts out to stop the doors with his hard on.

    I laughed at first. Funny situation, but looks really scripted.

    Fake, he would both be in excruciating pain and legitimately fearful of permanent injury. Also, every dude has negative mental imagery (overflowing toilet, autopsy pictures) sufficient to suppress that and get the blood back to flowing.
