Mchale's inability to find the right rotations. Mchale's inability to form an offensive/defensive scheme. And iso ball with Harden. Off topic- If you're going to dribble the clock out then at least have the balls to take the low percentage shot. Don't pass the ball off and throw your teammates under the bus.
Offensive strategy was absurb...I'm sure most of us could formulate a strategy and diagram more advance plays than Hack McFail...I mean, what the heck makes him a BBall guru? That he had some great post moves, and played tough defense? Last time I checked, those skills don't really involve BBall IQ. I mean, Shaq was a great post player and played tough D...do you want him coaching your team?
Calls could have been worse, we didn't play well enough to win, really. 5 fouls early hurt, but we had our chances and made poor decisions with the basketball.

It's a young team. But the final 15 or so possessions were almost exclusively perimeter ISOs for Harden, Lin, Parsons. No effective picks and screens. I'd put it on McHale for inability to offer a viable option there.
