I've lived in Utah for 6 years now after moving from the SF Bay Area. I wasn't sure what to expect in a place where 9 of 10 are white. The vast majority of the people here are fine, but some are extremely bigoted and dont try to hide it.
Depends on what he said. Booing and talking trash to opposing players should be within the rights of fans. Saying offensive things about a player's race, family, health, etc. needs to be thrown out.

Yeah, they could sell that ticket to someone else, but companies, in general, try to keep good relations with their customers, especially the ones that have money. Besides, refunding him doesn't hurt the impact of throwing him out. Lots of people saw that if you heckle too hard, you get tossed, and they'll think twice. Meanwhile, no one but the sales rep and the fan (and his circle of friends) will know he got refunded. So, there's no harm in it.
