Joe Hart has done some amazing things this season and made some incredible saves, but that does look kinda fake, maybe the it's just the quality of the video. On another note, the commentator is just as impressive.

If you look at the bottom of the screen on the replay, there's a ref that's running almost as fast as he is, so I doubt its fake.
He was trying to help his team score. It's like in hockey when they pull a goalie to put on an extra skater near the end of the game. The game was almost over. It was in the 93rd minute. Regular season matches go for a full time of 90 minutes. If Manchester City scores and ties the game, both teams win a point. If Manchester United won the game, Man U gets the full 3 points while Man City gets no points. In their league, they don't have a playoff system. The team with the best record, (points) wins the League Championship.
I doubt it's fake though. I mean, it's ****ing Manchester United. I think everyone watches their game.
