Mayo and all of its derivatives are evil condiments plotting against me to destroy my lunch. If I'm not completely persistent one of them will slip by and ruin my day.
real swiss cheese, sauteed mushrooms, crispy bacon... and fried jalapenos or onion strings. Probably no sauces at all, they make it too messy.
Guacamole, some jalapenos, nice sharp cheddar cheese, some good bacon, thinly cut tomatoes...and then some good old spicy mustard.
Goat cheese, roasted red peppers, avocado, caremelized onions, pancetta, sauteed mushrooms deglazed with brandy, roasted garlic aioli... I think it's time for lunch.

with all due respect, and i like all the ingredients you listed, don't you think that burger has a little too much going on?
All you need is a think slice of pâté, a few black truffle shavings, and some gold foil flakes and this burger could be on Iron Chef
I just don't like plain burgers much, I guess. I rarely do burgers and when I do, I go all out and do a burger party. If I can't get it all to fit on one burger I'll do two and split the toppings. And foie gras would be a nice addition... gold foil doesn't taste like much. From my experience with truffles, you really want to use them when you can show off the flavor - truffled mashed potatoes, for example.
Avocado, mixed greens tossed in balsamic vinegar, and a very small amount of plain yellow mustard. I just had a veggie burger, dressed as described. Only thing that could make it better would be some hummus.
Dirty Martin's on the drag in Austin has 2 burgers on the menu with fried eggs. Best burgers in the world.
I put peanut butter on the bottom bun of the burger so I guess it isn't a topping but it's still part of the burger combo and it's tasty.
