Like the other users said, it's like Oblivion with guns in the third person view, but there is Vats, and that makes a BIG difference in preserving some of the old fallout feel. I think if you liked Oblivion, you should like this game. It's not the same as the old Fallout, but I don't think we'll ever see a game like that again. This game is a little different, but still have some traces of the Fallout lineage left, enough for me to like it.
Thinking about picking this up, but am hesitant due to the Oblivion comparisons. Does this game have load time issues as well?
I actually managed to run through the full primary mission once already. If you want to play a while, hold off on the primary quest because the game ends after you complete it, and once you get near the end of the quest, the nature of the changes means you kind of have to keep working on it.
I kinda miss the pretty scenery...but on the whole this game is way more fun than Oblivion/Morrowind. VATS is the jam

Very cool about the GECK coming out and downloadable content. I don't have the most time in the world to devote to video games, but any time I do have has been devoted to this game recently. It's really a lot of fun. The more I get into it, the more fun it has become. The graphics sometimes used on the railway rifle used in VATS are pure genius. I got this game the day it came out, and still have a ton left to do.
