They don't bite it off, they use their mouth to suck blood from the wound. That's how the disease is getting in there. The guys spit is going right into the blood stream.
I guess maybe it was as good as anything back in the old days, before modern medical technology. But these are not the old days, and we do have better instruments to do that with (if you're going to do it). The wine in the mouth is obviously not a perfect sterilizing agent... doesn't work all the time. Besides, no offense but the whole in-the-mouth thing seems pretty perverted by today's standards...
