Difference between Ultram/Tramadol and hydrocodone on paper is that Ultram is not considered a narcotic and not a DEA controlled substance. Most Docs will be more liberal in prescribing it and be happy to see that you are on Ultram rather than narcotics such as hydrocodone. In reality, ultram may also have some addiction potential.
Definitely get a diagnosis as meggoleggo pointed out because treatment is different for different types of arthritis.
The other thing that's worth trying is Glucosamin/Chontroitin which is not a drug and a food supplement availabe OTC and have proven effect on common arthritis. These substances normally exist in your joint so presumably they act by supplying the joint re-building materials. Won't hurt at least.

Yeah, I'm taking the Osteo Biflex as well as some other thing that I forgot the name. I just got back from the doc. Got another Cort shot, but this time, in the front of the shoulder...SO MUCH FUN!!!

At least it is starting to feel better now. He may have found something that works, because the cort shots to the back of the shoulder only marginally reuced the pain. The shot to the front was more painful, but the actual shoulder pain is already going away.
