How Cocaine Makes Users Skinny

We already knew cocaine was a hell of a drug, but apparently it helps you lose body fat. You can eat more and have less body fat thanks to cocaine.

Cocaine has long had a reputation for keeping users slim—check out this 1990 article from the Los Angeles Times—and there's some science to back that up, too. Now, a new study of cocaine-addicted men examines how, exactly, cocaine makes users thinner.

Cocaine can fundamentally alter the body's metabolism, the study found. Even though addicts eat more and have altered protein levels that should make them gain weight, they're actually likely to have less body fat than non-addicts.

Knowing this could help doctors manage the weights of those who stop using cocaine, the research team, four neuroscientists from the University of Cambridge in the U.K., wrote a paper they published in the journal Appetite. Many of those who quit cocaine gain a lot of weight and reducing that effect could keep people happier with abstinence and more likely to stay on the wagon.

Cocaine addicts reported eating more calories, fat and carbohydrates. Yet the addicts actually had less body fat than non-addicts.

The research team studied 65 men from around Cambridge. Thirty-five of them were active, addicted cocaine users. On average, they had used cocaine for 15 years in either powdered or freebase form. The rest of the men were matched in age to the addicts, but they had never used cocaine and didn't have any history of substance abuse. The majority of the cocaine users, on the other hand, were also dependent on other substances such as opiates, alcohol and cannabis. Ninety-one percent of the cocaine addicts were dependent on nicotine.

Compared to men who weren't addicted to anything, the cocaine addicts reported eating more calories, fat and carbohydrates. Yet their body mass indices—a measure of people's weights compared to their heights—were the same as non-addicted peers of the same age. The addicts actually had less body fat than non-addicts, while both groups had the same amount of lean body mass. That result was especially striking because tobacco smokers usually have more body fat than non-smokers of the same height and weight, yet cocaine seems to overcome that: Remember, 91 percent of the cocaine users in this study were addicted to tobacco, too.

Cocaine addicts had higher levels of leptin, a protein associated with weight gain and appetite control, than non-addicts. The difference wasn't statistically significant, but it suggests something different is going on in addicts' metabolisms, the Cambridge researchers wrote. Cocaine may interfere with people's ability to store fat, which would explain why addicts are leaner, but also want to eat more fatty foods.

No.

Look at the data. The cocaine guys are eating fattier food, but they seem to have less body fat, a lower heart rate, and lower blood pressure.

Habits of skipping breakfast were more frequently reported in cocaine-dependent men (86%) compared with healthy men (20%) (χ2 = 28.2, p < 0.001). The cocaine users also reported a higher intake of alcohol (t42.5 = −2.5, p = 0.017) and caloric energy (t61.1 = 6.9, p = 0.033), as measured by the FFQ, but which were statistically controlled for in the analysis of dietary data. Overall, cocaine users’ diets differed significantly from those of their non-drug using peers (F56,6 = 11.2, p = 0.003; Wilks’ Lambda = 0.003). Specifically, cocaine users’ consumed significantly more fatty foods (F1,61 = 19.8, p < 0.05), as reflected by increased levels of both monounsaturated (F1,61 = 15.9, p < 0.05) and saturated acids (F1,61 = 22.6, p < 0.05). The cocaine group also reported consuming significantly more carbohydrates compared with the control group (F1,61 = 15.7, p < 0.05), but less sugar such as fructose (F1,61 = 16.8, p < 0.05) and glucose (F1,61 = 16.9, p < 0.05). Subsequent analysis with tobacco smoking status and prescribed medication both included as additional covariates did not change the results. It is noteworthy that the reported dietary intake did not differ between cocaine-dependent men with and without opiate dependence or with and without alcohol dependence, respectively (all p > 0.1).

Multiple regression analysis showed that the fat content of the diet explained 91% of the variance of the overall energy intake in the cocaine group (R2 = 0.907, F1,33 = 332.85, p < 0.001), but not in the control group. Carbohydrates explained 94% of the consumed dietary calories in the diets of the healthy volunteers (R2 = 0.944, F1,28 = 485.94, p < 0.001).

Besides the differences in dietary intake, cocaine-dependent men also reported differences in the pattern of food consumption compared with the control volunteers, as measured by the TFEQ. Cocaine-dependent men reported exerting less control over food intake (F1,63 = 11.6, p = 0.001) and less dietary restraint to influence their body weight (F1,63 = 6.9, p = 0.011) compared with their healthy peers. The inclusion of tobacco smoking and prescribed medication did not change the significance on both measures. There was also no evidence that co-morbid dependence on opiates and alcohol affected the results.
Explorative relationships between personality traits and eating patterns

As shown in Fig. 1, impulsive and compulsive personality traits were differentially associated with eating behavior in the two groups. Consistent with the literature, impulsive personality traits were significantly associated with uncontrolled eating in healthy volunteers (r = 0.47, p < 0.001) (Yeomans, Leitch, & Mobini, 2008), but no such relationship was seen in the cocaine group (r = 0.20,p = 0.282). Uncontrolled eating in the control group was further associated with individuals’ dietary fat intake (r = 0.34, p = 0.006), but this relationship was only marginally significant in the cocaine group (r = 0.30, p = 0.078). By contrast, restrained eating to influence body weight was significantly associated with compulsive personality traits in the cocaine group (r = 0.43, p = 0.016); but compulsivity was not related to fat consumption in either group (cocaine: r = −0.02, p = 0.646; control: r = 0.17, p = 0.194). Trait compulsivity was marginally associated with compulsive cocaine use (r = 0.34, p = 0.068), but not with eating behavior.

I'd like to lose some weight, I thought diet and exercise were the answer when all along I just needed to consult my dealer. Stupid doctors, putting dealers out of business since 1850.

Doctors were prescribing cocaine and other stimulants to patients for centuries before modern medicine.

Now big pharma makes you pay 10x the price for the same thing.
