That part is always up to speculation. Certainly the plans that led up to Robb's death were ongoing well before the leeches were thrown into the fire. It seems unlikely that they wouldn't have happened if the leeches had never been thrown in the fire. So did she cause it to happen, or did Mellisandre just see that it was going to happen in the flames? Or was it just coincidence like a primitive witch doctor who performs a human sacrifice for rain, and if it rains then it "worked" and if it didn't rain, then they need more sacrifices.

Or like when the modern man prays to "God" for recovery from some sort of medical condition, for example, and if the person prayed for recovers then it "worked" and "God is great", and if the person dies then "God works in mysterious ways". So it took GRRM 6 yrs to write book 5? I'm pretty sure during those 6 yrs he's been working on 5,6 and probably 7. He continued letting on that he is rushing out book 6 because of the tv series are catching up. I'm pretty sure he knew they were doing a tv series so that 6 yrs delay was to get 6 and 7 fleshed out.
The delay came mostly (or at least allegedly) because the story timeline that GRRM wanted to tell in the 4th book stretched so long and included so much there was no way it could fit into one book. So instead of finding a good cliffhanger/stopping point and splitting it chronologically, he split it according to areas/characters. So both books share a large amount of chronological timeframe, but are told from different characters perspectives. I think the idea sounded good to GRRM, but he quickly realized what a mess he had made. It made the writing of books 4 and 5 really, really painstaking (there was a huge delay between books 3 and 4 just like there was between books 4 and 5). Now that he is through that ordeal and is simply focusing on a writing in the style he did for books 1, 2 and 3 it should go faster. My concern is that the dude has a lot of distractions now (including the HBO show) so I worry that writing the books has become a chore for him and he is taking so long because his heart really isn't into it anymore. We'll see.
Yeah, his explanation for splitting books 4 and 5 the way he did was that he'd rather tell the whole story story of half the characters than tell half the story of all the characters. So book 4 and the first part of book 5 occur at the same time and certain characters don't appear at all in book 4 while other characters don't appear in book 5 until it catches up to the point in time where book 4 ends.
Yeah, given the way that books 4 and 5 ended up being structured, that gives HBO all the more reason to forge a different path through the story. They're certainly not going to allow losing whole character groups for an entire season. One can hope that in the same way book 3 will be two seasons, they'll also feel free to do more seasons for the content of 4 and 5 than just two. I would think there are probably three seasons of material in those two books (if not four seasons). And if they have more seasons Martin will have more time to catch up.
It's my understanding that HBO will combine books 4 & 5 for purpose of the series so we don't lose the characters we are following for a year.
