    Bad coaching. OKC had the most turnovers in the NBA last year. Sam Presti has a policy to have his point guard play through turnovers in order to learn what works and what doesn't. Punishing players for turnovers is just really stupid coaching.

    NBA wasn't specifically trying to make the Rockets bad, they were just planning the schedule so that the best teams with the most marketable players had the most favorable scheduling. Rockets are collateral damage, all the other teams have to take the hit when you favor Miami/OKC/etc.
