simmering feud---regarding H1B Visas---among MAGA allies

    This will impact the medical field more than anything. There is a shortage of all doctors in the US, leading to more mid-level providers being tasked with being "physicians" (with sub-par training and less experience).

    Yes, you could lower the standards to become a doctor... but doesn't that interfere with the mindset of allowing unqualified individuals to be granted spots based on factors other than competence?

    You could ask more US citizens to go into the medical field... and spend the 10+ years of education/training to get there.... (while some continue to attack the value of 'higher education').

    Or you could live with the shortage, the longer time it takes to see doctors/specialists, continued limitations/access issues in rural areas, continued quality concerns compared to other developed nations, continued price escalations due to those shortages (and greedy hospital systems/insurance companies), and continued escalations in the cost of medicaire/medicaid due to all of these factors.

    Access to health care (and training HCW) should never be a "political" issue.... and yet everybody will pretzel themselves into a corner trying to defend one or the other side of it. The constant is having enough people trained in this country to be able to handle the shortage, and these actions will put that into flux.

    Foreign Physicians should not be training on H1Bs. They should have a J1 or rarely to never O1. If they want to stay, then they do two years in a medical underserved area and then go for permanent residence and citizenship.

    We have had a physician shortage for decades and will have one until the administrators and insurance companies declare that midlevels (no offense to you NPs) are good enough. But H1Bs are not a temporary or permanent solution for this shortage.

    Section 1. Restriction on Entry. (a) Pursuant to sections 212(f) and 215(a) of the Immigration and Nationality Act (INA), 8 U.S.C. 1182(f) and 1185(a), the entry into the United States of aliens as nonimmigrants to perform services in a specialty occupation under section 101(a)(15)(H)(i)(b) of the INA, 8 U.S.C. 1101(a)(15)(H)(i)(b), is restricted, except for those aliens whose petitions are accompanied or supplemented by a payment of $100,000 — subject to the exceptions set forth in subsection (c) of this section. This restriction shall expire, absent extension, 12 months after the effective date of this proclamation, which shall be 12:01 a.m. eastern daylight time on September 21, 2025.
    (b) The Secretary of Homeland Security shall restrict decisions on petitions not accompanied by a $100,000 payment for H-1B specialty occupation workers under section 101(a)(15)(H)(i)(b) of the INA, who are currently outside the United States, for 12 months following the effective date of this proclamation as set forth in subsection (a) of this section. The Secretary of State shall also issue guidance, as necessary and to the extent permitted by law, to prevent misuse of B visas by alien beneficiaries of approved H-1B petitions that have an employment start date beginning prior to October 1, 2026.
    (c) The restriction imposed pursuant to subsections (a) and (b) of this section shall not apply to any individual alien, all aliens working for a company, or all aliens working in an industry, if the Secretary of Homeland Security determines, in the Secretary’s discretion, that the hiring of such aliens to be employed as H-1B specialty occupation workers is in the national interest and does not pose a threat to the security or welfare of the United States.

    So it's a pay for play scheme.

    J1 is quite risky lengthy path for Physicians, (mandatory exit, probation period) most of PA in Canada are former J1( weren't able to finish their residency due to visa expiration or no renewals which also diminish their ability to find residency acceptance in Canada as well)

    now for those who are qualified with all USMLA steps ,100k ain't much as a matter of fact it just become easier to secure h1b with less competition ,but who really want to move to the US at this time !
