[Urgent Photoshop Help] Trying to Raise Money for Charity - Need 1400x1400 Artwork

Hey guys, I am a pharmacist and also a 2nd year medical student. We are trying to raise money for the Lubbock Impact/TTUHSC Free Clinic. They provide healthcare to the poor and homeless at NO COST. They operate on donations and would be very grateful for some more funds as their budget is tight.

Originally, I had planned to do only a med school parody music video on youtube, but it's just such a great opportunity to raise money since it will generate 100,000+ views. I'll will also be on the local radio/tv stations to promote this. The president at TTUHSC is on board with this project. The song is called I'm so Nerdy, a parody of Fancy by Iggy Azalea. I'm going to release this song on iTunes and give 100% to the Free Clinic. The video is actually done, but I can't release it until I get the audio approved on iTunes.

All I need is someone w/ basic photoshop skills to make a 1400x1400 album cover for iTunes. White background, black text. I just need some frames (already have image) on bottom 3/5 of the page, and on the top, put "Super Guo", with "I'm So Nerdy" (should be slightly bigger than "Super Guo") below it. Font in Helvitica or Futura (Arial if you don't have them). That's it. You will be credited on the song and on youtube.

JPG file type
72 - 300dpi (300dpi is the best)
Less than 25mb
RGB color scheme (not CMYK)
Unique to this particular title. We cannot accept artwork that has been used for another title.

You may NOT upload artwork that displays the following:
Website URL
Contact information (i.e., email address, phone number)
Pornographic images
Pricing information
Copyrighted images
Scan of a CD (must be retail-ready artwork)
Blurry or pixelated images
