Sorry, I wasn't alive for the Miracle on Ice so I didn't list it. Still, I would count that as more of an "upset" than a "chokejob"

2004 ALCS out of the choices because being 3-0 and losing 4 straight is crazy. 18-1 really wasn't a choke, the Giants just had more luck on their side. Butler interception was preceded by a crazy catch from Seattle. Without that catch, there wouldn't even be a Butler INT. But really honestly, the two that are up there from recent memory are 2011 World Series game 6 with 2 outs and a 2-2 count -- 1 strike/1 out away from winning a WS and the 2013 NBA Finals with one rebound away from winning an NBA title. Both of those series the team that lost game 6 end up losing game 7 and lost their championship.
